Harbor Health Elder Service Plan Social Workers provide supportive care to our Program of All-inclusive Care for the Elderly participants and their caregivers every day, both at the Day Center and in their homes. To celebrate National Social Work Month our Social Workers are reaching out to family members caring for aging parents, older adults providing care for loved ones affected by progressive diseases like Parkinson’s, Alzheimer’s, or Diabetes and caregivers looking for resources to keep their aging family members at home.
