About Elder Service Plan.
Elder Service Plan (ESP) of Harbor Health Services, Inc. is a Program All-inclusive Care for the Elderly (PACE) model of care. The PACE concept, developed in 1973 in San Francisco, addresses the concerns that elders wanted to stay in their homes and communities; that many elders were not getting the services to make them successful at home; and that they frequently fell through the cracks, due to the fragmentation of the health care system.
PACE's goal is to maximize the health and functioning levels of participants, enabling them to continue living in the comfort of their own homes and in the community.

Welcome to the Elder Service Plan of Harbor Health’s website. We are excited to tell you about our PACE (Program of All-Inclusive Care for the Elderly) model of care.
PACE brings to frail elders and their family caregivers a comprehensive, integrated team model of care that works so well for elders faced with chronic conditions, multiple medications, home-based care needs and the coordination of all these challenges and more.
Our team is made up of: Geriatric Physicians, Nurse Practitioners, Social Workers, Physical and Occupational Therapists, Dieticians, Nurses, Recreational Therapists, Personal Care Attendants. Their goal is to bring their specific skills to a dynamic and team-based effort to keep elders at home, in the community, for as long as possible.
Because we are the providers of the medical care covered by Medicare and Medicaid, our team has flexibility in creating the most individualized, appropriate plan of care for our participants. You’ll be amazed by our success stories as shared on this site.
