The Harbor Health Elder Service Plan, (ESP), is centered on the belief that it is better for the well-being of seniors with chronic care needs and their caregivers to be served in the community whenever possible.

Harbor Health ESP provides healthcare coordinated by a team of medical, social, and behavioral health professionals. You and your family are an active part of this Caring Team which includes primary care physicians, registered nurses, social workers, case managers, physical and occupational therapists, dietitians, adult day center professionals, home care and transportation staff.

What Makes Pace Different?

Harbor Health ESP, a PACE model of care is a unique program for chronically ill elders. The medical care at Harbor Health ESP is provided by an interdisciplinary team collaborating daily at the Harbor Health ESP Day Center to provide participants with coordinated, effective medical care. Daily meetings at the Day Center allow our clinicians to creatively address the care needs of the participants and to develop and implement Care Plans to address the unique medical needs of our participants.
Most of the healthcare is provided by our Care Teams at the Harbor Health ESP Day Center or the comfort of the participant’s home, when necessary. Harbor Health ESP provides access to services 24 hours a day, seven days a week, and 365 days a year.

All Medicare and Medicaid benefits are covered in full, without copayments, through Harbor Health ESP. Covered services include:

  • Primary, Preventative, and Specialty Medical and Nursing Care
  • Hospital and Emergency Care
  • Behavioral Health and Social Work
  • Occupational and Physical Therapy
  • Dietician
  • Adult Day Center and Homecare
  • Transportation for the Day Center participants and Medical Appointments
  • Prescriptions and medicine management programs
  • Short and long term rehabilitation and care
  • Home Care and End of Life Care

Upon enrollment participants must receive all health care – other than emergency services – as authorized by Harbor Health ESP, or be fully and personally liable for costs of unauthorized services.

Program Features

Medical Services: Harbor Health ESP physicians, specially trained in gerontology, provide primary care to our participants and coordinate referrals to medical specialists when needed. Registered nurses provide participants with a personalized preventive healthcare program.
Behavioral health staff provides sensitive and caring support for emotional, family and social issues.

Registered dietitians at Harbor Health ESP know how vital a healthy, nutritious diet is to the health and well-being of our participants. If it is needed, Harbor Health ESP provides personally tailored and monitored meal planning and special diets.

Emergency, urgent care, and ambulance service is available through Harbor Health ESP. Should a higher level of care be required, Harbor Health ESP will arrange for hospitalization, nursing home care, or additional home-care services.
Harbor Elder Service Plan provides physical and occupational therapy to support participants living successfully in their homes and communities.

Physical Therapy — therapeutic exercise, fitness training and massage.

Occupational Therapy — making the home environment safe and ease to use, assistance with activities of daily living, and improved self-sufficiency.

Registered nurses provide participants with a personalized preventive healthcare program and monitor the participant’s progress with regard to their healthcare.

As a Harbor Health ESP participant, prescription drugs are ordered by your ESP primary care physician and provided by ESP – eliminating the need to use an outside pharmacy. Required laboratory, diagnostic and x-ray services, and durable medical equipment and supplies may also be provided.
Personal care assistance in the participant’s home is available when prescribed by the participant’s primary care physician and can be an essential part of independence for our participants. The care provided is task orientated and generally not appropriate for individuals needing 24 hour care to remain at home safety.
Harbor Health ESP provides a safe and caring environment specifically for ESP participants. Specialized Day Center Programming includes:
• Aromatherapy
• Music and Memories certified program
• Music and art therapist
• Painting classes
• Massage therapy
• Mind Body Connection including Meditation, Tai Chi, chair and Laughter Yoga

Empowering and uplifting the lives of our participants through meaningful recreational activities, support and socialization. All of our programs aim to elevate the well-being of our participants by exposing them to activities that meet their physical, mental, spiritual and emotional needs.

Harbor Health ESP provides transportation for participants to and from the ESP Day Center along with transportation to all medical appointments either at the ESP Day Center or other medical facilities as needed.
