The Harbor Health Elder Service Plan, (ESP), is centered on the belief that it is better for the well-being of seniors with chronic care needs and their caregivers to be served in the community whenever possible.
Harbor Health ESP provides healthcare coordinated by a team of medical, social, and behavioral health professionals. You and your family are an active part of this Caring Team which includes primary care physicians, registered nurses, social workers, case managers, physical and occupational therapists, dietitians, adult day center professionals, home care and transportation staff.
What Makes Pace Different?
Harbor Health ESP, a PACE model of care is a unique program for chronically ill elders. The medical care at Harbor Health ESP is provided by an interdisciplinary team collaborating daily at the Harbor Health ESP Day Center to provide participants with coordinated, effective medical care. Daily meetings at the Day Center allow our clinicians to creatively address the care needs of the participants and to develop and implement Care Plans to address the unique medical needs of our participants.
Most of the healthcare is provided by our Care Teams at the Harbor Health ESP Day Center or the comfort of the participant’s home, when necessary. Harbor Health ESP provides access to services 24 hours a day, seven days a week, and 365 days a year.
All Medicare and Medicaid benefits are covered in full, without copayments, through Harbor Health ESP. Covered services include:

Program Features
Behavioral health staff provides sensitive and caring support for emotional, family and social issues.
Registered dietitians at Harbor Health ESP know how vital a healthy, nutritious diet is to the health and well-being of our participants. If it is needed, Harbor Health ESP provides personally tailored and monitored meal planning and special diets.
Emergency, urgent care, and ambulance service is available through Harbor Health ESP. Should a higher level of care be required, Harbor Health ESP will arrange for hospitalization, nursing home care, or additional home-care services.
Harbor Elder Service Plan provides physical and occupational therapy to support participants living successfully in their homes and communities.
Physical Therapy — therapeutic exercise, fitness training and massage.
Occupational Therapy — making the home environment safe and ease to use, assistance with activities of daily living, and improved self-sufficiency.
Registered nurses provide participants with a personalized preventive healthcare program and monitor the participant’s progress with regard to their healthcare.
