Eligibility.
Harbor Health ESP enrolls seniors living with health challenges that prevent them from living independently, qualifying them for "Nursing Facility Care", (nursing home), as determined by the State of Massachusetts. Enrollment is voluntary, which also allows a participant to withdraw from the program at their own discretion.
We serve the communities in Avon, Boston, Braintree, Brookline, Canton, Dedham, Milton, Newton, Norwood, Quincy, Randolph, Sharon, Stoughton or Weymouth
Costs.
Some individuals use a combination of the coverages listed. For those eligible for Medicaid, there may be a monthly spendown, due to financial criteria. Our enrollment team can assist you to determine your eligibility. Please call for a complete financial overview and evaluation.
There are no co-payments for ESP medical and support services. If the participant's health status changes, the private payment (spendown amount) remains the same.
Upon enrollment participants must receive all health care – other than emergency services - as authorized by Harbor Health ESP, or be fully and personally liable for costs of unauthorized services.

Enrollment Process.
2 – Home Visit
A home visit will be made by an Enrollment Specialist. During this time, the Medicaid, (Mass Health), application process will be started, if necessary.
3 – First Visit
We will arrange for the potential participant to come to the ESP Day Center for a formal screening by a nurse and social worker. The formal screening assesses whether or not the individual qualifies as needing "Nursing Facility Care", (nursing home). Family members are asked to participate in this visit. If not done already the Medicaid, (Mass Health), application process may be started during the first site visit.
4 – Second Visit
A second site visit will be scheduled for the potential participant to visit the Day Center. This second visit will include time to participate in Day Center activities and enjoy a tasty lunch. The family is not required to attend, if needed transportation will be provided.
5 – Approval
The formal screen must be approved by the State of Massachusetts and the Harbor Health ESP team will determine whether or not a participant will be safe in the community.
6 – Finalize
Meet with an enrollment representative to do the "signing in" for the program. Enrollment into the program begins on the first day of the following month.
