What is the Harbor Health Elder Service Plan (PACE)?

For the last 21 years, Harbor Health Elder Service Plan has provided a Program of All-inclusive Care for the Elderly (PACE) in the Greater Boston and recently Southeastern Massachusetts area. This comprehensive package of customized services developed to meet the needs older adults with chronic medical conditions. Our mission is to help keep older adults healthy while experiencing personal independence by living in their own home and community.

This model of medical and health care not only provides participants the care they require, but supports the families and caregivers as well.

Services can include primary medical care, home health services, rehabilitation programs, transportation medications, hospitalizations, meal programs, durable medical equipment and even family caregiver support. In addition, Harbor Health Elder Service Plan provides Adult Day Health programs.

Once evaluated, we develop a customized Care Plan to meet the participant’s needs. The Care Plan is developed in union with the participant and their caregivers. A team of physicians, nurse practitioners, registered nurses, social workers, dieticians, physical and occupational therapists, pharmacists and other health care professionals coordinate the Care Plan. The team meets regularly with the participant and their family to adapt the plan to meet any changing needs.

Older adults, age 55 years and up with chronic medical conditions making is a challenge to live at home safely may qualify for the program. Our service area includes the Greater Boston and southeastern Massachusetts.
